IMPORTANT INFORMATION FOR PARENTS AND GUARDIANS

Required Immunizations:

Indiana State law requires all students to have a current immunization statement on file with the school. See below for the 2026-2027 required immunizations. Parents should call their family’s physician or the Putnam County Health Departmetn at (765) 658-2782 to make an appointment before school starts.

The Religious Objections to Vaccinations form must be completed annually. If you need a new form, find the link below or pick one up from one of the school offices. Please return the completed form to your student’s school.

Medications:

Students are not permitted to carry prescription medication while on the school bus or at school. Exceptions may be made when a parent/guardian provides the school with a signed statement from a physician indicating that the student may carry an inhaler, EpiPen, or insulin.

Parents/guardians must drop off prescription medications to the school and pick them up at the end of the school year. Any prescription medication that is not picked up at the end of the school year will be properly disposed of.  

All prescription medications must be in the original pharmacy-labeled container.  

Cloverdale Community Schools also provides a selection of common over-the-counter medications and basic first-aid products in the nurse’s office for students, with parent/guardian consent. Consent is provided through Harmony during registration at the beginning of each school year.

Available items include Tylenol (acetaminophen), Ibuprofen, Benadryl, Anti-Itch Cream, Sterile Eye drops, and triple antibiotic ointment. Cough drops are available for students 3^rd grade and above, and TUMS are available for students 12 years of age and older. 

Health Conditions:

If your child has any diagnosed health conditions such as astha, diabetes, seizures, or allergies requirring and EpiPen, it is very important to call or come in and meet with your school nurse at the beginning of the school year, or anytime your child has a change in condition.

Health Concerns:

As the Cloverdale Community School Corporation Nurse, my top priority is the health, safety, and well-being of every student while they are at school. To ensure that your child receives appropriate care in the event of an illness, injury, or an emergency, it is important that parents/guardians complete the health history section with accurate and current information.

Medical information may be shared with appropriate school personnel on a need to know basis to help protect your child and ensure their safety while at school.

Mandatory Hearing and Vision Testing:

Hearing tests will be given yearly to students in grades 7 and 10, and/or by the request of a student or teacher. Vision tests will be given yearly to students in grades K, 3, 5, and 8, and/or by the request of a student or teacher. Parents who do not want their child tested, please send in a written note stating that request.
